Equipment handover receipt
Every time you hand over a laptop, keys or protective equipment, the person receiving it notes what they get, the serial number and its condition, and signs on their phone. You get every handover in a table, with its PDF receipt, so you know who has what and can ask for it back.
Who it's for: Businesses, offices, warehouses, clubs and schools.

When to use it
- You hand out laptops, phones, keys or access cards.
- You hand out protective equipment and want a record of each handover.
- You want to know who has what so you can ask for it back.
What's inside and why
- Condition when received
- Avoids arguments on return: it's on record how it was.
- Commitment
- A heading with the commitment, right before the signature: each receipt records what was agreed by signing.
Tips to get the most out of it
- Fill in the serial number on handover: it tells you which item each person has.
- Download the CSV to keep the handover log in a spreadsheet.
- For returns, duplicate the form and change the questions: you'll have a record of each return too.
Frequently asked questions
What kind of signature is it?
A simple electronic signature: the typed name and, if the signer likes, a drawn signature, with the server's date and time. It's not an advanced or qualified signature; for an important contract or dealings with a public authority, use a qualified signature.
What does the signer get?
A PDF receipt with their answers, their signature, the date and time, the SHA-256 fingerprint of the record and a code to check it. You download the same receipt from each response.
What if I change the text later?
The receipt shows what the signer saw: the text of each question is saved when they respond. If you change the form later, earlier signatures don't change.
