School trip consent form

No more slips lost in school bags: parents read the trip details, give consent and sign on their phone by typing their name and, if they like, with a finger. Each family downloads a PDF receipt with the time and a code to check it, and you see who has given consent and who hasn't. It's a simple electronic signature, not a qualified one.

Who it's for: Schools, parent associations, music schools and sports clubs.

When to use it

  • You're organising a school trip and need consent from every family.
  • You want to see at a glance who has given consent and who hasn't.
  • You want each student's allergies and contact phone to hand during the trip.

What's inside and why

The trip
A heading with the trip details: it's what parents consent to, and it's saved as it was in every receipt.
Parent or legal guardian's signature
The typed name is the signature; drawing it with a finger is optional. With it, each family downloads their receipt.

Tips to get the most out of it

  1. Write the date, destination, times, transport and price in “The trip”: it's what is signed.
  2. Download the CSV the day before: you have the phone numbers and allergies in a table to print.
  3. Delete the responses when the trip is over: allergies are health data and there's no need to keep them.

Frequently asked questions

What kind of signature is it?

A simple electronic signature: the typed name and, if the signer likes, a drawn signature, with the server's date and time. It's not an advanced or qualified signature; for an important contract or dealings with a public authority, use a qualified signature.

What does the signer get?

A PDF receipt with their answers, their signature, the date and time, the SHA-256 fingerprint of the record and a code to check it. You download the same receipt from each response.

What if I change the text later?

The receipt shows what the signer saw: the text of each question is saved when they respond. If you change the form later, earlier signatures don't change.
