Photo and video consent
Before posting a photo, ask for permission in writing: each person (or their parent or guardian) chooses where their images may be used and signs on their phone. You get the list of who agreed to what, and each person gets a PDF receipt. Adapt the text to your activity and your privacy notice.
Who it's for: Schools, clubs, associations, event organisers and businesses.

When to use it
- You'll take photos or videos at an activity and want to post them.
- You need to know who agreed to what before sharing an image.
- You work with children and their family gives consent.
What's inside and why
- Where may we use the images?
- Three clear levels: all channels, internal communications only or nowhere. So you know what to do with each photo.
- Is the person under 18?
- If so, a parent or legal guardian signs.
Tips to get the most out of it
- Say in the heading which activity it is and who will take the photos.
- Download the CSV before posting: sort it by use and you have the list of who must not appear.
- Set up the form's privacy information: it's shown when people respond.
Frequently asked questions
What kind of signature is it?
A simple electronic signature: the typed name and, if the signer likes, a drawn signature, with the server's date and time. It's not an advanced or qualified signature; for an important contract or dealings with a public authority, use a qualified signature.
What does the signer get?
A PDF receipt with their answers, their signature, the date and time, the SHA-256 fingerprint of the record and a code to check it. You download the same receipt from each response.
What if I change the text later?
The receipt shows what the signer saw: the text of each question is saved when they respond. If you change the form later, earlier signatures don't change.
