Consent forms and approvals signed online, with a PDF receipt

For what used to be a paper to sign: a school trip consent, a photo consent, a quote acceptance or an equipment receipt. Respondents type their name and, if they like, sign with a finger; they get a PDF receipt with what they signed, when, and a code to check it. It's a simple electronic signature, not a qualified one.

What it does

The name is the signature
It's typed on the keyboard (works with screen readers and without JavaScript); a drawn signature with a finger or mouse goes with it if they like.
PDF receipt
With the answers, the signature, the date and time, the SHA-256 fingerprint and a QR code: the signer downloads it on submitting, and you from each response.
What was signed, saved
The text of each question is saved on responding: if you change the form later, earlier signatures don't change.
Checked in one click
The receipt's code opens a page that says whether it's genuine and shows the fingerprint of the record.
Who's missing
In the table and the summary, who has signed; in the CSV, the name with “✍” if they also drew their signature.
No overpromising
A simple electronic signature (eIDAS Regulation, Art. 25) and the server's time, not a qualified timestamp: the receipt itself says so.

How it works

  1. Pick a template (school trip, photo consent, quote…) or add the “Signature” question to your form.
  2. Write what is being signed in the heading: it's saved in every receipt.
  3. Share the link or QR code: each person signs on their phone and downloads their receipt.

Frequently asked questions

What kind of signature is it?

A simple electronic signature: the typed name and, if they like, a drawn signature, with the server's date and time. It's not an advanced or qualified signature.

So what is it for?

To keep a record, for everyday matters, of who responded, what and when: consent forms, permissions, approvals and receipts. For an important contract or dealings with a public authority, use a qualified signature.

How is a receipt checked?

With its QR code or code: the page says whether it matches a recorded response and shows its fingerprint, which must match the one on the PDF.

Is it free?

Yes, with no ads and no cap on responses or signatures. The only paid extra will be removing the AIM FORM badge.
